- Incorporation Certificate: Please provide an official document issued by a public institution that shows the company name and registered company address.
- Sole Proprietorship: Please provide an official document issued by a public institution that shows the applicant’s name and registered address.
2: Personal identification of the company representative or applicant (one of the following)
- Passport
- Driver’s License
- ID Card

3: Personal identification of the Ultimate Beneficial Owner (one of the following)

5: Bank Account
Enter the account information where sales proceeds will be credited.
*If you do not enter the correct information, we will not be able to remit the sales proceeds.
*If reassembling occurs due to the wrong information about your new bank account, the reassembling transaction fee and bank transfer fee will be charged to you. Please double-check to make sure everything is correct on the filled-out form.
